Our physicians require complete information to render an evaluation and treatment plan.
Please hand-carry to your appointment, any and all records, including imaging studies,  that are pertinent to your evaluation.  Images (MRIs, CTs, etc) must be reviewed at your appointment, on disc or on film.   A report alone is not sufficient.  Please understand that it may be necessary to reschedule your appointment if films are not available to review at the time of your appointment.
