Intracranial pressure

(ICP)

Pressure of the brain and other intracranial contents.

Monro Kellie model.  The pressure of the cerebrospinal fluid (CSF) and brain tissue is the same.

Measurement

Intracranial pressure can be measured by:

Lumbar puncture - pressure of cerebrospinal fluid (and brain) can be measured anywhere in the CSF space, including that surrounding the spinal cord and cauda equina.

Ventriculostomy - a catheter placed directly into the ventricle (also drains cerebrospinal fluid)

Parenchymal intracranial pressure monitor: a catheter placed into brain tissue (cannot be used to drain cerebrospinal fluid)

Monitoring

Continuous measurements allows charting of changes in intracranial pressure over time (intracranial pressure dynamics).

Control

Modalities used for control and reduction of intracranial pressure include:

Medical - diuresis, hyperventilation, and pharmacologic coma pentobarb coma

Surgical - removal of brain tissue

Cerebral perfusion pressure

Ventriculostomy insertion
