Our physicians require complete information to render an evaluation and treatment plan. Please, hand-carry to your appointment any and all records, including imaging studies, that are pertinent to your evaluation. Images (MRIs, CTs, etc) must be reviewed at your appointment, on a disc or a film. A doctor's report alone is not sufficient. Please, understand that it may be necessary to reschedule your appointment if films are not available to review at the time of your appointment.
